NASPGHAN MOC Transition
This $150 module will evaluate adequacy of participant documentation of how they prepare patients for the process of transition in relation to expert-endorsed standards.
Eligible participants include clinicians who see patients who will at some point require transition on a frequent enough basis to report data on 30 patients over a year period. NOTE: Patients can be at various stages during the transition process.
This activity will provide CME and 25 MOC Part IV credits within your 5-year certification cycle to participants who successfully meet completion criteria.
This activity will take a minimum of four (4) months to complete.
